California doctors’ dubious coronavirus claims condemned by health experts

A widely shared local television video of last week’s news conference, which was posted on YouTube, reached more than 5 million views and was amplified by Elon Musk and Fox News, where Dr. Dan Erickson and Dr. Artin Massihi appeared on primetime shows two nights in a row.

The video has since been taken down by YouTube for violating the platform’s policy on misinformation, a YouTube spokesperson said.

The doctors, who are not epidemiologists and who own and operate urgent care centers in the Bakersfield area, held the news conference on April 22 to share their conclusions about the results of 5,213 coronavirus tests at their clinics, extrapolating their findings to the California population as a whole.

“Do we need to still shelter in place? Our answer is emphatically no. Do we need businesses to be shut down? Emphatically no. Do we need to test them and get them back to work? Yes, we do,” Erickson said at the news conference.

Widespread condemnation

The comments and conclusions of the doctors drew widespread condemnation from health officials and medical experts.

“There is a lot to object to from a scientific point of view,” Andrew Noymer, an epidemiologist, told CNN. He said one big problem is that the doctors made estimates based on their clinics’ clients who were tested, not a sampling of the general population.

“What these doctors are doing is corrupting the process from the start to make it seem like they are doing an honest policy analysis,” added Noymer, who is an associate professor of population health and disease prevention at the University of California, Irvine.
The video also prompted the American College of Emergency Physicians and American Academy of Emergency Medicine to issue a forceful joint statement on Monday calling the pair’s claims “reckless and untested musings” that “are inconsistent with current science and epidemiology regarding COVID-19.”

“As owners of local urgent care clinics, it appears these two individuals are releasing biased, non-peer reviewed data to advance their personal financial interests without regard for the public’s health,” the statement read, “COVID-19 misinformation is widespread and dangerous. Members of ACEP and AAEM are first-hand witnesses to the human toll that COVID-19 is taking on our communities. ACEP and AAEM strongly advise against using any statements of Drs. Erickson and Massihi as a basis for policy and decision making.”

Health officials in Kern County, where Bakersfield is located, also said they disagreed with the claims made by Erickson and Massihi.

“In our ongoing effort to mitigate the impacts of COVID-19 on our residents and healthcare system, we continue to adhere to the guidance issued by Governor Newsom regarding the stay at home order,” a Kern County health official said in a statement to CNN.

Erickson and Massihi did not return CNN requests for comment.

Va. Gov. Ralph Northam to allow non-emergency doctor, dentist visits

Northam (D) said he was lifting the ban on elective surgeries he imposed five weeks ago, allowing doctors, dentists and veterinarians to resume seeing patients on a non-emergency basis beginning Friday.

That ban, which applied to hospitals, outpatient surgery centers, dental offices and veterinarians amid concerns that the state’s hospitals would be overwhelmed by a surge in coronavirus cases, was originally scheduled to expire last week, but Northam extended it to May 1.

On Wednesday he confirmed that he would not extend it further, touting the decision as proof that the commercial and social restrictions he has imposed have successfully slowed the spread of the virus.

Northam said the change also was possible because of the work that state and medical officials have done to secure more capacity for treating and testing coronavirus patients. But he warned that if cases surge again, it might be necessary to reinstate restrictions.

Meanwhile, Sen. Chris Van Hollen (D-Md.) pleaded with Senate Majority Leader Mitch McConnell (R-Ky.) to impose safeguards when the chamber resumes its work in Washington next week to avoid risking the spread of the virus.

In his letter, Van Hollen said that recalling Senate lawmakers and staff members to Capitol Hill without precautions that maximize telework, minimize support staff, and institute temperature checks and rapid testing would undermine stay-at-home orders imposed across the District, Maryland and Virginia.

“While the Senate has critical work to do on behalf of the American people, we must be attentive to our role in controlling the spread of COVID-19 and have clear plans in place to ensure the safety of the staff and Capitol Police who support our work and care for the Capitol,” the letter states. “. . . I am ready to get back to the Capitol but you have an obligation to ensure the safety of all the staff that work there.”

McConnell’s office declined to comment beyond referring to a radio interview in which the senator said that “it’s essential for senators to carefully man our [stations] and support those folks who are out there on the front lines.”

Van Hollen called the D.C. region a “covid-19 hotspot.” The District, Maryland and Virginia on Wednesday reported 106 new deaths and 1,471 new infections — bringing their total number of coronavirus cases past 39,000 and total deaths to 1,818.

The District, which Van Hollen noted was shortchanged in a federal relief act, is scrambling to manage the pandemic — including limiting service for the Metro rail and bus lines that many Senate staffers use to commute to work.

D.C. officials said Wednesday that covid-19 restrictions and closures might need to be extended another three months under the “most stringent” scenario.

In a town hall Wednesday on reopening the city, District officials said a less stringent approach to reopening would phase in reopening in two to three months. The most stringent option would extend current closures for three months.

Do you have many patients with peri-implant mucositis? Do you know what is more important when treating these patients? Here are five tips:

1st Advice: Pay Special Attention To Risk Groups

Peri-implant mucositis is very frequent. Systematic work together shows that more than 63% of patients with implants consulted suffer from it. Smokers suffer especially. In the case of patients with total prostheses supported by implants, more plaque is usually found in the support elements than in patients with partial prostheses. Therefore, pay special attention to these patients at risk.

2nd Tip: Avoid Going Further

If peri-implant mucositis occurs, the situation may get even worse, and there is a risk of periimplantitis. To avoid this, it is necessary to reduce mucositis, since subsequent damage can be very serious. A measure of known efficacy is the mechanical removal of bacterial deposits. The additional use of selected preparations for the control of germs can contribute to the success of the treatment.

3rd Tip: Perform Periodic Reviews

Periodic reviews are essential in the case of patients with peri-implant mucositis. They check if the peri-implant tissue shows symptoms of inflammation and if there is a recession. It should also be carefully observed if plaque has accumulated in the implant.

Making bacterial deposits visible with a fluorescent indicator liquid (e.g., Plaque test) helps to advise the patient as to the causes. The risk areas, which require special attention during brushing, are clearly identified. The motivating success of dental hygiene is easily demonstrated. Similarly, you can explain the possibilities and usefulness of professional dental cleaning. Thus your patients will be more willing to invest in these measures.

4th Advice: Perform A Professional Cleaning

Periodic professional cleaning of the teeth should take care and maintain in the long term the quality of the surfaces of the more or less resistant materials and the hard tissue. This is true both in the case of implant treatments and for restorations or natural tooth substance. It is essential to avoid irreversible scraping with an unsuitable cleaner, either with prophylactic pastes or very abrasive cleaning powders. On the one hand, microorganisms settle earlier on rough surfaces. On the other, resin or ceramic restorations lose their aesthetic shine and tend more to discoloration.

5th Advice: Oral Care According To The Problem

The personal oral care is very important for oral health and long – term conservation of implant treatments. It is even more important when a mucositis has already occurred. Similarly, the depth of the users’ bags was reduced. During the entire application period, no stains or changes in flavor were observed. The use of the mild flavor gel was high. An oral care gel should be easy to use. Thus it can be easily integrated into the daily brushing routine.

Irwin Smigel, New York Dentist Behind Cosmetic Techniques, Dies at 92

Irwin Smigel, a pioneer of aesthetic dentistry who brightened the smiles of celebrities like Elizabeth Taylor, Tony Bennett, Jimmy Fallon and Bruce Willis, died on Monday at his home in Manhattan. He was 92.

The cause was complications of pneumonia, said his son, Robert Smigel, a comedian and screenwriter and the voice of the puppet Triumph the Insult Comic Dog.

Working from his office on Madison Avenue, Dr. Smigel (pronounced SMY-gell), a second-generation dentist, advanced two techniques that transformed the appearance of teeth in countless patients beginning in the 1970s.

One was laser whitening, in which a laser beam activates a chemical bleaching agent applied to the teeth; Dr. Smigel helped develop and popularize it.

The other was a bonding technique, which had been used for fillings; he applied it to dental imperfections using layers of laminates and veneers.

Dr. Smigel credited a number of colleagues, among them Dr. Michael Buonocore and Dr. Raphael Bowen, with also making breakthroughs in the bonding process.

In bonding, instead of gold or porcelain caps being applied, the tooth is etched with a buffered acid solution and then painted with a liquid acrylic. A composite resin is then patted on, shaped and hardened for 20 seconds with a beam of either white or ultraviolet light.

Bonding was originally known as acid-etching, Dr. Smigel told Global Health Nexus, a publication of the New York University College of Dentistry, in 2004. But he stopped using that term in 1975, he said, after he was interviewed by the New York radio personality Long John Nebel.

Recalling that interview, Dr. Smigel said: “During a commercial break, he said: ‘People will be frightened by the thought of acid used in their mouths. Why not call it bonding instead?’ And it’s been known as bonding ever since.”

Dr. Smigel once demonstrated the technique on “The Mike Douglas Show,” enlisting a man from the television studio audience to undergo it.

Dr. Smigel attracted patients from all over the world and had continued to practice until about two years ago.

He and his wife, Lucia Smigel, who survives him, also developed a whitening toothpaste called Supersmile.

“The eyes may be the soul of the face, but the mouth is the first thing people look at,” Dr. Smigel told New York magazine in 1981. “Now bonding has given us the possibility of instant transformation, since it can be done in a matter of hours, not days and years.”

He added, “Nothing will have the emotional impact on the public that bonding will.”

Irwin Elliot Smigel was born on Oct. 9, 1924, in Manhattan, the son of Dr. Sebastian Smigel and the former Bella Soloway. He graduated from Stuyvesant High School, New York University and the N.Y.U. College of Dentistry.

In addition to his wife, the former Lucia Shvetz, and his son, he is survived by a daughter, Bellanca Smigel Rutter, a lawyer for Supersmile, and six grandsons.

As, I write my last president’s message, I have been reflecting on the last 27 months. Thank you for the amazing opportunity you gave me to earn your trust and support. My last president’s message will be a call to action. Our specialty is under attack from every possible angle… insurance companies, the threat of advanced practice provider (APP) independent practices, CMGs controlling our practice, congress “fixing” the balanced billing issue, etc. 

As a specialty, we need for all emergency physicians to get involved. I often hear: “Why should I join an AAEM committee?” “Why should I write and visit my congressional representative?” “Why should I encourage others to join AAEM and get involved?” “Why should I donate to the AAEM PAC?” “I am content, I am happy where I am!” This is the “laissez-faire” attitude that got us where we are at today! Laissez-faire is a French noun meaning “attitude of letting things take their own course, without interfering” We allowed others to make decisions for us, we allowed others to be at the table making decision for us—not just emergency physicians, but all physicians.
